Qualifications

• LVN/LPN license is required.
• Track record of connecting with people quickly and building trust.
• Comfort in a high-volume, fast-paced outbound calling environment.
• Familiarity with Motivational Interviewing techniques or structured conversation frameworks.
• Strong active listening skills and documentation ability.

Responsibilities

• Drive a high volume of outbound calls daily and connect with patients.
• Build genuine rapport with your patient panel and maintain ongoing engagement.
• Learn and apply risk stratification to focus outreach and reduce client disengagement.
• Set PCP visit goals and drive improvements in health outcomes, adherence, and navigation.
• Translate conversations into clear actions and clinical documentation for the care team.
