Job summary

Qualifications

• Completion of at least one college-level health or science course exposing to healthcare concepts or terminology.
• Customer service, patient-facing, healthcare-related, or administrative experience is a plus.
• Current or prior CNA/MA-like experience is preferred but not required.

Responsibilities

• Support the Patient Coordination team in outreach calls to patients post-discharge to ensure follow-up appointments and adherence.
• Assist patients with non-clinical needs, including navigation of resources and referrals.
• Collect and document patient information in designated systems accurately.
• Monitor and record patient concerns, escalating clinical issues as appropriate.
• Provide compassionate communication and education under supervision about care plans and self-management.
• Support administrative tasks like organizing records and scheduling appointments.
• Collaborate with healthcare team to resolve patient needs and coordinate services.
• Adhere to policies, confidentiality, and compliance protocols.
