Qualifications

Responsibilities

• Conduct outreach calls to patients post-discharge to ensure follow-up.
• Assist with non-clinical needs and navigation of healthcare resources.
• Document patient information accurately in designated systems.
• Monitor and escalate patient concerns to licensed staff as needed.
• Support scheduling and administrative tasks to coordinate care.
