Job summary

SCP Health is seeking a student-focused Patient Coordination team member to support post-discharge outreach, ensure follow-up appointments, and assist with non-clinical navigation. You will document patient information and escalate concerns when needed while working with licensed staff to reinforce care plan instructions.

This remote role emphasizes compassionate communication, patient education, and collaboration with the healthcare team, all while adhering to privacy and compliance standards.

Qualifications

• Completion of at least one college-level health or science course providing exposure to healthcare concepts or medical terminology.
• Customer service, patient-facing, healthcare-related, or administrative experience is a plus. Current or prior experience as CNA/MA or similar role is preferred but not required.

Responsibilities

• Support the Patient Coordination team in conducting outreach calls to patients post-discharge, ensuring follow-up appointments, medication adherence, and patient understanding of discharge instructions.
• Assist patients with non-clinical needs, including navigation of healthcare resources and referrals.
• Collect and accurately document patient information in designated systems.
• Monitor and record patient-reported concerns, escalating clinical issues to licensed staff as appropriate.
• Provide compassionate communication and patient education under supervision, reinforcing instructions related to care plans and self-management.
• Assist with administrative tasks, including organizing records, maintaining data accuracy, and supporting patient scheduling efforts.
• Collaborate with healthcare team members to ensure timely resolution of patient needs and service coordination.
• Adhere to organizational policies, patient confidentiality requirements, and compliance protocols.
