Job summary

Pine Park Health seeks a Remote Registered Nurse with an active California RN license to support older adults from a home office, serving California, Nevada and Arizona through a tech‑enabled primary care model.

You will collaborate with PCPs and Medical Directors, triage patient communications, prepare charts, educate patients, and develop care plans. You’ll oversee post‑visit orders and participate in daily huddles for coordinated care.

Qualifications

• Active California RN license required.
• Minimum 2 years of clinical experience, geriatrics preferred.
• Strong communication and accountability in remote settings.
• Experience with geriatric patients in senior living or SNF settings.
• Familiarity with chronic care management and geriatric diseases.

Responsibilities

• Collaborate with PCPs and Medical Directors to support a large patient panel.
• Triage inbound communications (calls, faxes, email, messages) and route tasks appropriately.
• Support LVN/LPNs and act as escalation point within the team.
• Prepare charts and provide patient education on care plans.
• Oversee post-appointment processes, including orders and referrals.
• Participate in daily huddles to promote teamwork.
• Conduct virtual patient assessments and develop care plans.
• Serve as a communication link among patients, PCPs, and specialists.
• Promote preventive care and self-management aligned with care plans.
