Job summary

Acentra Health is seeking a Clinical Evaluator (PASRR) in Nebraska. The role conducts PASRR Level II assessments across hospitals, nursing homes, and community centers, determining level-of-care recommendations while ensuring compliance with regulations.

This PRN hybrid position offers remote work from home with local travel and fully reimbursed travel expenses. The role requires an active Nebraska healthcare license (RN or other LMHP) and experience in mental health assessment.

Qualifications

• Active Nebraska license in healthcare or related discipline (LCSW, LMSW, LMFT, LICSW, LCPC, LCP, RN with psychiatric specialty, or LMHP).
• RN candidates must hold current, unrestricted RN licensure.
• Master's degree in behavioral health or related field.
• 3 years of experience in assessment/diagnosis of mental/psychiatric disorders.
• Minimum 2 years working with individuals with intellectual/developmental disabilities or related conditions.
• Proficiency in medical record abstraction and review.
• Strong clinical judgment and evidence-based assessment.
• Excellent interpersonal, verbal, and written communication skills.

Responsibilities

• Complete PASRR Level II assessments by reviewing records and evaluating physical, behavioral, and psychological functioning.
• Develop level-of-care recommendations and determine next level of care for long-term placement.
• Review medical records and provide clinical recommendations per regulatory requirements.
• Interact with healthcare professionals, patients, families, and internal partners as needed.
• Use technology and software platforms to manage assessments and track activities.
• Ensure compliance with workflows and timely completion within scope of practice.
• Read and adhere to corporate policies including HIPAA rules.
