Job summary

Acentra Health is seeking a Clinical Evaluator - PRN to perform face-to-face PASRR Level II assessments across nursing homes, hospitals, and community settings in Kansas. You will determine the appropriate levels of care for individuals with SMI, ID, or RC, with travel to Topeka or Wichita and surrounding areas.

The role combines remote work from a home office with local travel, offering flexibility and meaningful impact on patient outcomes.

Qualifications

• Must hold an active Kansas license in one of the following: LCSW, LMSW, LMFT, LICSW, LCP, RN, or LMHP.
• Minimum of 2 years of experience assessing individuals with mental health or psychiatric disorders.
• Minimum of 2 years of experience assessing and analyzing psychological, emotional, behavioral, and physical functioning, with knowledge of Level of Care standards.

Responsibilities

• Conduct PASRR assessments and develop level-of-care recommendations across settings.
• Adhere to standards of mental health, ID/RC, geriatrics, and related nursing care.
• Ensure compliance with contractual and regulatory standards in care planning.
• Use clinical judgment and communication to navigate complex cases for members and providers.
• Collaborate with healthcare professionals, patients, families, and support networks to meet timelines.
• Stay informed on regulations, long-term care, and geriatrics through literature and updates.
• Utilize state electronic systems and proprietary platforms to track assessments.
• Maintain high quality and efficiency by following established workflows.
• Read and adhere to corporate policies including HIPAA.
