Job summary

Acentra Health is seeking a PASRR Clinical Evaluator - PRN to join our Sacramento area team. This role conducts PASRR level II assessments, makes level-of-care recommendations, and ensures that care quality meets regulatory standards.

The position is a flexible PRN with a hybrid mix of telehealth and in-person assessments in nursing facilities and hospitals in the Sacramento region. California licensure and mental health assessment experience are required to deliver compliant, patient-centered

Qualifications

• Active, unrestricted California licensure in healthcare or related field (LPCC/LMHC/LMFT/LCSW/NP/PMHNP)
• Graduation from an accredited college or university with a degree in a relevant field
• 3+ years of experience in assessment/diagnosis of mental/psychiatric disorders
• 1+ years of experience with individuals with intellectual/developmental disabilities or related conditions

Responsibilities

• Perform PASRR Level II assessments by reviewing records and gathering information
• Develop evidence-based level-of-care recommendations
• Review medical records and provide clinical recommendations meeting regulatory standards
• Interact with healthcare professionals, patients, and families to support assessment
• Leverage technology and software platforms to manage assessments
• Ensure timely completion of assessments within scope of practice
