Job summary

Acentra Health is seeking a Clinical Evaluator - PRN to perform PASRR Level II assessments and determine appropriate levels of care for individuals with complex needs across nursing homes, hospitals, and community settings. This role emphasizes clinical expertise in SMI, ID, and related conditions.

The position is hybrid, with in-person preadmission evaluations in Topeka or Wichita and surrounding areas, plus remote work from your home office and local travel as needed.

Qualifications

• Active Kansas license in one of the approved LMHP roles (LCSW/LMSW/LMFT/LICSW/LCP/RN or equivalent).
• Minimum of 2 years of experience assessing individuals with mental health or psychiatric disorders.
• Minimum of 2 years of experience assessing and analyzing psychological, emotional, behavioral, and physical functioning.

Responsibilities

• Conduct PASRR assessments and evaluations across settings (nursing homes, hospitals, community).
• Develop informed level-of-care recommendations based on evaluations.
• Ensure compliance with state standards and regulatory requirements.
• Navigate complex cases with clinical judgment and strong communication skills.
• Collaborate with diverse healthcare professionals, patients, families, and support networks.
• Stay current with literature, regulations, and research related to long-term care and geriatrics.
• Utilize state electronic systems and proprietary platforms to manage assessment activities.
• Maintain attention to detail and adhere to established workflows.
• Read and adhere to HIPAA and Privacy/Security Rules.
