Job summary

Mercy is seeking an experienced Registered Nurse to join our team as Nurse Navigator (Part-Time, Hybrid). You will guide patients through their healthcare journey, coordinating care, improving outcomes, and ensuring a seamless experience from pre-treatment through follow-up care.

The Nurse Navigator collaborates with patients, families, physicians, bedside nurses, and interdisciplinary teams to assess needs, coordinate care plans, and connect patients with resources.

Qualifications

• Graduate of an accredited School of Nursing
• Current RN license in the state of practice
• BLS certification
• 2–3 years nursing experience

Responsibilities

• Coordinate care for inpatient and outpatient populations
• Guide patients through treatment journey as resource and advocate
• Provide patient and family education including pre-operative prep and follow-up
• Collaborate with physicians, nurses, and multidisciplinary teams
• Support discharge planning and post-acute services
• Monitor patient progress and improve outcomes and satisfaction
• Assist with LOS management and utilization review
• Participate in quality improvement and best-practice implementation
• Serve as a clinical resource and mentor to care team
• Maintain documentation in EPIC EHR
