Job summary

Mercy is seeking an experienced Registered Nurse to join our team as a Nurse Navigator. In this Part-Time, Hybrid role, you will guide patients through their healthcare journey, coordinate care, and support transitions from pre-treatment to follow-up care.

You will collaborate with an interdisciplinary team to assess needs, educate patients and families, and ensure high-quality, patient-centered care while contributing to discharge planning and utilization management.

Qualifications

• Minimum of 2-3 years of nursing experience.
• Current RN license in the state of practice.
• BLS certification required.

Responsibilities

• Coordinate care for inpatient and outpatient populations.
• Guide patients through treatment journeys and advocate for their needs.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and aim to improve outcomes and satisfaction.
• Assist with LOS management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to care team members.
• Maintain accurate documentation in EPIC.
