Job summary

Ennoble Care seeks an experienced Advanced Practice Provider (NP or PA) to deliver remote palliative care to patients across NJ, PA, GA, VA, OK, KS. You will apply medical knowledge under physician supervision to manage a diverse geriatric population with chronic conditions.

Qualifications

• Board-certified NP or PA with valid state licenses (NJ/PA/GA/VA/OK/KS).
• 2–3 years of palliative, chronic care management, or primary care experience with geriatric patients.

Responsibilities

• Provide high-quality palliative care to patients remotely across multiple states (NJ, PA, GA, VA, OK, KS).
• Perform physical examinations, review histories, and develop care plans.
• Communicate with clinic operations and staff across platforms.
• Collaborate with the care team to develop holistic plans of care.
• Lead conversations about goals of care and hospice transition.
• Order diagnostic tests as needed and prescribe meds and necessary interventions.
