Job summary

Ladders is seeking an Advanced Practice Provider (NP or PA) for a remote Palliative Care role to deliver patient-centered care to high-risk members. You will lead assessments, care planning, and symptom management with a strong emphasis on telehealth and coordination of transitions of care.

Qualified candidates have 5 years in advanced practice (3+ in palliative care), active U.S. licenses, ACHPN or willingness to obtain, and comfort with technology in a fast-paced environment.

Qualifications

• Current, active NP or PA license in at least one U.S. state.
• Willingness to obtain additional state licenses with provided support.
• 5 years in advanced practice, including at least 3 years in palliative care.
• ACHPN credential or willingness to obtain within six months.
• Comfortable with technology and a fast-paced environment.
• Strong interpersonal and communication skills.

Responsibilities

• Perform initial assessments and develop palliative plans of care for new members.
• Manage a panel of high-risk patients through regular telephonic and video encounters.
• Provide consultative visits for advanced symptom management and complex goals of care.
• Evaluate hospital admissions and support transitions of care to reduce readmission rates.
• Participate in regular team huddles and interprofessional meetings.
