Job summary
Devoted Medical Group, PC is seeking a highly motivated palliative care nurse practitioner to help build the clinical team for our virtual program serving patients with life-limiting illness across multiple states. Qualifications
- Must possess an active NP or PA license in at least one U.S. state.
- 5 years in advanced practice with at least 3 in Palliative care and ACHPN credential or willingness to obtain within six months.
Responsibilities
- Panel management and developing Palliative Plans of Care.
- Conduct telephonic/video encounters for high-risk patients.
- Provide consults to RNs and coordinate complex symptom management, goals of care, and hospice transitions.
- Collaborate with interprofessional care teams and leadership for seamless patient care.
- Attend Devoted Medical meetings and provide on-call coverage as needed.
- Support the team during rapid growth and PTO/absences.
