Job summary
Tuesday Health is seeking an Advanced Practice Provider to lead clinical care delivery in collaboration with senior leaders and clinicians. This remote role supports patients and families in palliative and serious illness care, coordinating with network providers across Texas.
The candidate will hold an MSN or PA degree, have 5+ years in palliative or hospice settings, and possess TX licensure, DEA, and board certification.
Qualifications
- MSN or PA degree required.
- Five+ years in palliative care, hospice, or serious illness care.
- Active TX RN or PA license with no board actions.
- Board certification as an APRN and active DEA license.
- Ability to coordinate with network and longitudinal care providers.
Responsibilities
- Lead internal and joint rounds to ensure optimal care plans.
- Coordinate services with network and longitudinal care providers.
- Review population data and implement data-driven interventions.
- Train clinical staff to share up-to-date knowledge across the team.
- Support quality management and quality improvement initiatives.
- Improve performance and quality of network partners with guidance.
- Collaborate with clinical, operational, financial, and technology teams.
- Monitor utilization, outcomes, satisfaction, and growth opportunities.
- Represent the company in external forums and stay current on trends.
