Job summary

Devoted Medical is seeking a skilled Advanced Practice Provider (NP or PA) to join its remote Palliative Care program across multiple states. You will manage high-need patients, perform initial assessments, and develop goals of care while coordinating with interprofessional teams.

A focus on telehealth delivery, documentation, and patient-centered care is essential. The role requires NP or PA licensure in at least one state, with licensing support for others.

Qualifications

• Must hold an active NP or PA license in at least one U.S. state.
• Minimum 5 years in advanced practice, with 3+ years in palliative care.
• ACHPN credential or willingness to obtain within 6 months; multi-state licensure support provided.

Responsibilities

• Perform initial assessments and develop Palliative Plans of Care for new enrollees.
• Conduct telephonic/video follow-ups for high-risk patients.
• Provide consultative visits to patients and coordinate care with RNs and other clinicians.
• Support transitions to hospice care and manage complex symptoms.
• Lead panel activities and participate in interprofessional team meetings.
