Job summary

Thyme Care is seeking an Oncology Social Worker to provide comprehensive psychosocial support to members and their care networks via phone, email, text, and telemedicine. Responsibilities include assessment, resource linkage, coordination with oncologists and nurses, advocacy, crisis intervention, and leading education/support groups.

This role reports to the Director of Social Work and can be remote or hybrid based in Nashville or New York City; all patient interactions are via our virtual care

Qualifications

• Master’s degree in Social Work from CSWE-accredited program.
• Valid state LMSW/LISW/LCSW licensure.
• OSW-C or APHSW-C certification preferred.

Responsibilities

• Assessment and psychosocial support for patients and families.
• Resource linkage to community services for complex social needs.
• Care coordination with healthcare team to ensure integrated care.
• Advocacy with insurers and providers to access services.
• Crisis intervention for urgent situations.
• Education and facilitation of cancer support groups.
