Thyme Care, Inc. is seeking an experienced social work professional to serve as the primary intake clinician for escalated behavioral health and social needs. You will conduct psychosocial assessments, provide brief interventions for stabilization, and coordinate care across care teams and community resources.

Qualifications

• MSW from a CSWE-accredited program.
• Active, unrestricted independent clinical social work license (LISW/LCSW) in current state.
• Minimum 3 years’ clinical social work experience in healthcare; crisis intervention and behavioral health preferred (oncology/palliative/serious illness care desirable).
• Proven competence in risk assessment and crisis management; telehealth/phone clinical skills.
• Excellent clinical judgment and ability to make time-sensitive decisions while working remotely.

Responsibilities

• Serve as the primary intake clinician for escalated behavioral health and social needs consults; complete psychosocial and safety assessments via phone, video, or ThymeBox.
• Deliver brief interventions for stabilization and safety planning.
• Conduct comprehensive psychosocial assessments addressing emotional concerns, financial toxicity, and caregiver needs.
• Coordinate with Nurses, Care Partners, Providers, and community organizations to address barriers to care.
• Maintain timely documentation of triage assessments, referrals, and escalations; provide clinical consultation to Care Team.
• Support knowledge-sharing and refinements of triage processes and pod handoffs.
