Job summary

Thyme Care is seeking a Clinical Social Worker to provide psychosocial and emotional support to our cancer care communities by phone, email, text, and telemedicine. You'll perform assessments, counseling, crisis intervention, and care coordination for patients and families.

The role reports to the Director of Social Work and can be remote or hybrid in Nashville or New York City. You’ll work with oncologists, nurses, and care teams to reduce distress and improve access to services.

Qualifications

• Master's degree in Social Work from CSWE-accredited program.
• Valid and current LISW or LCSW license in practicing state.
• OSW-C or APHSW-C certification preferred.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Deliver crisis intervention during behavioral health crises.
• Link patients to community resources for complex needs.
• Collaborate with oncology teams to ensure coordinated care.
• Advocate for patients with insurers and providers.
• Lead education sessions and cancer support groups.
