Included Health is seeking Oncology Nurse Care Managers for our Care and Case Management team to provide holistic, telephonic remote care for members. You will guide patients through complex medical and behavioral health situations, collaborating with a diverse clinical team to deliver integrated care from a distance.

The role emphasizes creating cohesive care plans, advocacy, and navigating benefits and resources while maintaining HIPAA compliance.

Qualifications

• BSN required with active RN license.
• Oncology experience highly preferred.
• 5+ years of nursing experience.
• 2+ years experience in care management.
• Must reside in a compact NLC state; remote role.

Responsabilités

• Deliver coordinated, patient-centered virtual Care Management by telephone or video.
• Create impactful care plans with members and the care team to achieve goals.
• Help members navigate complex medical conditions, treatment pathways, benefits, and healthcare system.
• Partner with members' local providers to ensure coordinated care.
• Provide compassionate, longitudinal follow-up care with supportive relationships.
• Assist through acute healthcare episodes with coordinated Case Management.
• Coordinate resources addressing clinical or social needs.
