Job summary

Daymark Health Inc. is seeking an Oncology Nurse Navigator to join an integrated Care Team for virtual and telephonic care delivery.

You will perform triage, case management, and care navigation for oncology patients with various comorbidities to improve quality of life. You will collaborate with a Medical Director, Nurse Practitioner, and Social Worker, ensuring adherence to clinical protocols and care pathways while maintaining strong patient relationships and documentation.

Qualifications

• Bachelor of Science in Nursing required with an unrestricted RN license; RI licensure or willingness to obtain is needed.
• At least 3 years of nursing experience, including 2 years in oncology.
• Oncology certifications such as OCN, AOCN, AOCNS, or CCM are strongly preferred or will be obtained within 2 years of hire.

Responsibilities

• Provide direct patient care through virtual platforms and deliver high-quality care to meet each patient’s needs.
• Perform triage and symptom management for oncology patients and escalate to Nurse Practitioner when needed.
• Coordinate care with Medical Director, NP, Social Worker, and other members of the care team.
• Educate patients about diagnosis, Daymark Health’s care offerings, and next steps.
