Qualifications

• Active, unrestricted RN license in state of residence.
• 3+ years of acute care hospital RN experience (medical-surgical, post-surgical, ICU, or similar).
• 3-5-year acute oncology experience required.
• ADN required; BSN preferred.

Responsibilities

• Conduct telephonic assessments to evaluate member healthcare needs.
• Develop individualized care plans to improve health outcomes.
• Coordinate services and programs for members with complex needs.
• Apply clinical expertise to identify risks and facilitate transitions.
• Partner with multidisciplinary teams to optimize member outcomes.
• Engage members with strong communication and motivational interviewing.
