Job summary

ConnectMed360 is seeking a Patient Support Case Manager to guide patients through enrollment, access, and ongoing therapy across pre- and post-prescription stages. This role can be fully remote.

You will coordinate with providers, caregivers, and client teams while delivering patient education and navigating benefits, prior authorizations, and program materials. Confidential handling of patient information is essential.

Qualifications

• Minimum of 5 years of pharmacy experience required.
• Associate degree or equivalent program preferred.
• Certifications in PACS, CHES or CCM are desirable.

Responsibilities

• Serve as primary contact for patients, caregivers, and healthcare providers.
• Provide enrollment, access and program-related support throughout the patient journey.
• Educate patients with resources, FAQs, and approved materials.
• Maintain accurate and confidential patient records in the system.
• Coordinate with pharmacies and client departments to ensure timely therapy.
