Qualifications

• Registered Nurse with current California license or equivalent (state license valid to practice).
• Experience in hospital or clinical settings with complex chronic diseases preferred.
• Care Management experience strongly preferred; oncology patient experience preferred.
• Strong verbal and written communication; collaborative teamwork.
• Proficiency with Microsoft Office and basic computer skills.

Responsibilities

• Coordinate clinical care with health care staff to plan, implement and monitor patient care.
• Manage chronic disease care, including oncology, ensuring compliance with regulations.
• Document encounters accurately in the designated system and maintain records.
• Participate in interdisciplinary case conferences and care coordination activities.
• Coordinate transitions and discharge planning, ensuring patient education and follow-up.
