Included Health is seeking Oncology Nurse Care Managers for our Care and Case Management team. In this telephonic role, you will guide members through complex medical and behavioral health situations, partnering with a diverse clinical team to deliver integrated remote care.

You will advocate for members, create cohesive care plans, and navigate benefits to support longitudinal health outcomes. The position requires a BSN, 5+ years nursing experience, and 2+ years in care management, with

Qualifications

• BSN required and RN licensed.
• Oncology experience preferred.
• 5+ years in nursing.
• 2+ years in care management.
• Must reside in a compact NLC state.
• Willingness to obtain multi-state licensure.

Responsibilities

• Deliver remote, patient-centered care management by phone or video.
• Create cohesive, actionable care plans with members and care team.
• Assist with navigation of complex medical conditions, benefits, and system pathways.
• Coordinate with local providers to ensure care continuity.
• Provide longitudinal follow-up and compassionate engagement.
• Support during hospitalizations and acute episodes with case management.
