Highmark Health is seeking a seasoned Case Manager to provide comprehensive case management services, coordinating care for members while controlling costs and promoting optimal health outcomes. The role requires clinical expertise, strong assessment skills, and collaboration with physicians and care teams.

You will develop and monitor individualized care plans, advocate for members, ensure documentation compliance, and mentor junior case managers in a fast-paced, compliant healthcare

Formación

• 5 years in any combination of clinical, case/utilization management, and/or disease condition management experience.
• Oncology, Transplant, Maternity, NICU, Pediatric) experience or exposure.
• Experience working with the healthcare needs of diverse populations.
• Advanced training in CBT, motivational interviewing, or DBT.
• Expertise in clinical assessment and care planning.
• Proficiency in using Microsoft suite and case management software/EHR.
• Strong understanding of healthcare systems and case management principles.
• Bachelor's in Nursing.

Responsabilidades

• Manage a caseload of members with complex health needs.
• Conduct comprehensive assessments and develop complex care plans.
• Assess members’ health status, needs, and available resources.
• Develop individualized care plans with members, physicians and providers.
• Implement and coordinate care plans, ensuring access to services.
• Monitor member progress toward goals and adjust plans as needed.
• Advocate and liaison to meet individual health care needs using communication and resources.
• Collaborate with physicians, providers, and health care team to carry out the plan.
• Ensure all activities are documented and compliant with policies and regulations.
• Provide guidance and mentorship to lower-level Case Managers.
• Serve as a resource for clinical expertise and problem-solving.
