Job summary

Cardinal Health’s Individualized Care Nurse Care Management role provides structured, patient-centered services that coordinate medical treatments, education, and follow-up. You’ll bridge patients, providers, and support services to maintain continuity of care in a remote setting.

The position requires an RN with a CA license and about five years of hospital/clinical experience in chronic disease management, with oncology experience being a strong plus.

Qualifications

• Registered Nurse with a current, unrestricted California license.
• 5 years’ hospital or clinical experience with complex chronic disease states preferred.
• Care Management experience is strongly preferred; Oncology patient experience is a plus.
• Proficient in Microsoft Office; familiarity with EHR systems and data entry.

Responsibilities

• Coordinate care and collaborate with onsite clinical staff to support discharge planning and care transitions.
• Document encounters accurately in the EHR and maintain HIPAA compliance.
• Identify trends and coordinate with oncology providers and care management teams.
• Provide patient education, medication reconciliation and self-management coaching.
• Manage a caseload of 100–150 members and escalate complex cases as needed.
