Job summary

You will collaborate with physicians and a care team to ensure access to services, while containing costs and adhering to HIPAA and compliance requirements. This role includes mentorship of junior staff and ongoing professional development.

Qualifications

• 5 years of clinical, case management, or disease condition management experience.
• Experience with Oncology, Transplant, Maternity, NICU, or Pediatrics.
• Experience serving diverse populations.
• Advanced training in CBT, motivational interviewing, or DBT.
• Expertise in clinical assessment and care planning.
• Proficient with Microsoft Office, EHRs, and case management software.

Responsibilities

• Manage a caseload of members with complex health needs and advanced clinical knowledge.
• Conduct comprehensive assessments and develop complex care plans.
• Assess health status, needs and available resources.
• Collaborate with members, physicians and other providers to implement care plans.
• Coordinate care plans and ensure access to services and resources.
• Monitor progress and adjust care plans as needed.
• Advocate for members to promote quality, cost-effective outcomes.
• Collaborate with physicians and healthcare team.
• Document activities in compliance with policies and regulations.
• Provide guidance and mentorship to junior Case Managers.
• Serve as a resource for clinical expertise and problem-solving.
