The Care Team Hospice is seeking an On-Call Registered Nurse (RN) to provide after-hours support for hospice patients and families in Ohio. This role emphasizes independent clinical decision-making, compassionate, one-on-one care, and the option to stay at home when not actively responding to patient needs.

Four overnight on-call shifts weekly, with coverage ending Friday morning and no Friday evening, weekend on-call, or daytime caseloads.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of one year of nursing experience; hospice/home health preferred.
• Strong clinical assessment, symptom management, and critical-thinking skills.
• Ability to work independently and communicate compassionately.

Responsabilités

• Respond to after-hours patient and caregiver needs with timely nursing assessments.
• Complete scheduled and urgent visits, addressing changes in condition.
• Provide symptom management and end-of-life care with empathy.
• Educate on medications and collaborate with the interdisciplinary team.
• Document accurately in the EMR and ensure continuity of care.
