Job summary

One Senior Care Inc. seeks an On-Call Registered Nurse to provide after-hours clinical support to Participants, caregivers, and authorized representatives.

You will assess concerns, guide care decisions, coordinate services, and connect Participants with resources. This role requires independent work with access to designated providers and escalation resources to ensure safe, timely support and thorough follow-up communication with the care team.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active RN license in every state where assigned.
• Minimum of one year of professional nursing experience.
• Experience with frail older adults, geriatrics, PACE, home-based care, triage, emergency or urgent care, primary care, or care coordination is strongly preferred.

Responsibilities

• Answer the designated on-call line during assigned coverage periods.
• Identify Participant, caller, location, reason for call, and urgency of concern.
• Complete focused nursing assessments based on symptoms, history, medications, and caregiver input.
• Recognize changes in condition and emergencies requiring escalation.
• Provide clear nursing guidance within scope of practice and care plans.
• Coordinate next steps including monitoring, provider consultation, or home-based services.
• Document all calls and dispositions in the EMR with accuracy.
