Job summary

One Senior Care Inc. is seeking an On-Call Registered Nurse to provide after-hours clinical support to Participants, caregivers, and authorized representatives.

You will assess concerns, provide guidance within the RN scope, and coordinate care using established protocols. This role offers remote, after-hours coverage with opportunities to collaborate with a dedicated care team, focusing on safety, continuity, and patient advocacy across OSC centers.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active RN license in every state assigned.
• Minimum of one year of professional nursing experience.
• Experience with frail older adults, geriatrics, PACE, home-based care, triage, or care coordination preferred.
• CPR/BLS certification.

Responsibilities

• Answer the designated on-call line during assigned coverage periods.
• Identify the Participant, caller, location, reason for the call, and urgency of the concern.
• Complete focused nursing assessments based on symptoms, history, medications, and functional status.
• Recognize changes in condition and escalate emergencies or urgent concerns as needed.
• Document all calls and nursing actions in the electronic medical record or approved system.
