Cassia is seeking an experienced RN for After-Hours triage in Minnesota. The role is on-call and remote, with required licensing and potential in-person training in Edina. You will triage calls from clinical staff, coordinate care with hospitals and families, and document all activities in the EMR.

The position requires Assisted Living experience and a Director of Health Services background is preferred but not required. Training may occur at Cassia sites, and Cassia will cover related expenses.

Qualifications

• Must have a current Minnesota RN license and be in good standing.
• Willing to come to the Edina corporate office for 1 day of training and 1-5 days of training at Cassia sites.
• Prior RN experience in Assisted Living or long-term care.
• Director of Health Services experience preferred, but not required.
• Must have strong communication, organizational and critical thinking skills.
• Experience with electronic medical records and learning new technology; reliable internet.
• Self-starter to join the RN triage team delivering continuity of care after hours.

Responsabilités

• Triage incoming after-hours calls from clinical staff regarding urgent resident situations.
• Assess, prioritize and provide clinical guidance for timely care.
• Coordinate with hospitals, providers, families, and site leadership as needed.
• Act as an extension of site DHS to support clinical decision-making and continuity of care.
• Document all calls, assessments, interventions, and outcomes in the EMR.
• Monitor and respond to calls throughout the entire shift.
• Provide clear, compassionate communication to staff, residents, and families.
• Build trust through responsiveness and clinical expertise.
