Job summary

Cassia is seeking an experienced Minnesota-licensed RN to join the After‑Hours RN team. This remote, part‑time role triages clinical calls, coordinates care, and supports site DHS decisions for Assisted Living residents in Minnesota.

You will respond to emergencies, document interactions in EMR, and collaborate with hospitals, families, and staff. The position requires training visits to Edina and various sites as needed.

Qualifications

• Current Minnesota RN license in good standing.
• Willingness to travel to Edina office for training and meetings.
• Prior RN experience in assisted living or long-term care.
• Experience in Assisted Living settings preferred.
• Director of Health Services experience is a plus.
• Strong communication, organizational, and critical thinking skills.
• Proficiency with electronic medical records and tech readiness.
• Self-starter and reliable team member for after-hours triage.

Responsibilities

• Triage after-hours calls from clinical staff for urgent resident situations.
• Assess and prioritize to guide appropriate, timely care.
• Coordinate care with hospitals, families, providers, and site leadership.
• Act as an extension of site DHS to support clinical decisions.
• Document calls, assessments, interventions, and outcomes in EMR.
• Monitor and respond to calls throughout the shift.
• Communicate clearly and compassionately with staff, residents, and families.
• Build trust through responsiveness and professional conduct.
