Job summary

Signature Performance is seeking an Observation Medical Coder for a remote/nationwide role in the United States. The candidate will code E/M encounters with ICD-10-CM/PCS, CPT, HCPCS, and modifiers from medical records, ensuring accurate DRG/APC assignment and compliant coding practices.

Qualifications

• Minimum 2 years of Medical Coding experience.
• Experience with Professional Fee Coding and EHR systems.
• Certifications from AHIMA or AAPC are accepted: RHIT, RHIA, CCS, CCS-P, CPC.

Responsibilities

• Assign principal and secondary diagnoses and procedures by reviewing documentation.
• Review discharge summaries, notes, radiology, labs, and operative records to assign codes.
• Ensure correct code selection following coding guidelines and regulations.
• Sequence diagnoses/procedures by clinical significance to determine DRG/APC/payment tier.
• Review coding for accuracy before submission to billing.
• Abstract required information and correct data in the system.
• Identify missing documentation and request physician queries to complete coding.
