Signature Performance is seeking an Observation Medical Coder to work remotely nationwide. You will apply E/M ICD-10-CM/PCS, CPT and HCPCS coding to encounters, ensuring compliant and accurate reimbursement. The role requires proficiency with medical record review, documentation analysis, and coding guidelines across payers.

Qualifications

• Minimum 2 years of Medical Coding experience.
• Experience with Professional Fee Coding.
• Experience with EHR systems.
• Accepted AHIMA or AAPC certifications include RHIT, RHIA, CCA, CCS, CCS-P, CPC.

Responsibilities

• Assign principal and secondary diagnoses and procedures by reviewing medical records.
• Review discharge summaries, notes, radiology, labs to assign codes.
• Ensure correct code selection following coding guidelines and regulations.
• Sequen ce diagnoses/procedures by clinical significance to determine DRG/APC.
• Review coding accuracy before submission to billing.
• Abstract data from records and enter into the system accurately.
• Identify missing or conflicting documentation and query physicians as needed.
• Meet coding quality and quantity expectations.
