Community Health Network is seeking an Ambulatory Intake Registered Nurse to join the OB/GYN Care team. The role involves handling incoming calls, triaging needs, and coordinating initial visits with referrals to appropriate providers.

The nurse will perform scheduled remote visits and coordinate care using standard protocols. The position emphasizes evidence-based patient education, virtual care coordination, and collaboration with care teams.

Qualifications

• Graduate of NLN/CCNE/ACEN accredited nursing program.
• Licensed RN in Indiana.
• Three+ years nursing experience; OB/GYN or L&D preferred.
• EPIC experience a plus.

Responsibilities

• Respond to incoming calls in a timely manner and triage to address caller needs.
• Perform scheduled remote visits and coordinate referrals to healthcare providers.
• Enter initial orders (labs, ultrasound) for services being established.
• Coordinate with Care Coordinators and Nurse Navigators for patient needs.
• Provide virtual coordination of care to patients, families, and providers.
• Deliver evidence-based pregnancy education and promote wellness.
• Schedule initial in-person visits and assist with related tasks as needed.
