Job summary

Community Health Network in Indianapolis is seeking an Ambulatory Intake Registered Nurse for the OB/GYN Care team. The nurse will handle calls, conduct remote visits, triage patient needs, obtain medical history, and coordinate referrals.

Initial labs and ultrasound orders are entered, and resources are directed via information systems. The role emphasizes evidence-based education, strong clinical judgment, and collaboration with Care Coordinators and Nurse Navigators.

Qualifications

• Graduate of NLN, CCNE, ACEN, or NLN CNEA accredited nursing program, or 3+ years of nursing experience.
• Licensed RN with valid Indiana license (NLC).
• Three years or more of nursing experience.
• Labor and Delivery experience required.
• OB/GYN experience highly preferred.
• Previous medical office/outpatient experience preferred.
• Phone triage experience a plus.
• Excellent communication skills.
• Excellent team skills.

• Accountable and self-directed.
• EPIC experience a plus.

Responsibilities

• Respond to incoming calls in a timely manner and triage to address caller needs.
• Perform scheduled remote visits and coordinate care.
• Obtain and assess patient history using standard evidence-based protocols.
• Provide education and recommendations to patients.
• Schedule initial visits and coordinate referrals to providers.
• Enter initial orders and referrals in the system.
• Refer patients to community resources using information systems.
• Coordinate with providers, Care Coordinators, and Nurse Navigators.
• Promote wellness and pregnancy education.
• Schedule the initial in-person visit and related tasks; remote work after onsite orientation.
