Job summary

Community Health Network is seeking an Ambulatory Intake Registered Nurse to join our OB/GYN Care team. The role handles timely calls, triage, and remote visits, using standard protocols to assess patient needs and coordinate care. Orientation is on-site with later remote work.

The ideal candidate has 3+ years nursing experience, Labor & Delivery and OB/GYN exposure, and Indiana RN licensure, with EPIC experience considered a plus.

Qualifications

• Graduate of NLN/CCNE/ACEN/NLN CNEA accredited nursing program or 3+ years of nursing experience.
• Licensed RN in Indiana per NLC.
• 3+ years nursing experience required.
• Labor and Delivery experience required.
• OB/GYN experience highly preferred.
• Previous medical office/outpatient experience preferred.
• Phone triage experience a plus.
• EPIC experience a plus.

Responsibilities

• Respond to incoming calls promptly and triage patient needs.
• Perform scheduled remote visits and coordinate initial visits.
• Obtain and assess patient medical history using standard protocols.
• Provide education and recommendations; refer to community resources.
• Enter orders and coordinate referrals with care team.
• Promote wellness and pregnancy education; remote coordination of care.
• Work remotely after onsite orientation.
