Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care by phone, addressing patients' medical, social, and psychological needs remotely. This full-time role supports 40 hours per week and requires an active RN license valid across OSH practice states.

You will provide confident service, document precisely, schedule appointments, arrange transportation, and collaborate to meet care and operational metrics from home.

Qualifications

• Active, non-probationary RN license valid in all OSH practice states.
• Proactive maintenance of licenses without prompts or audits.
• Ability to manage time and work autonomously with reliability.
• Focus on delivering an unmatched patient experience on every call.
• Motivation to complete CE requirements without prompting.
• Willingness to work flexible shifts incl. evenings, weekends, holidays.
• US work authorization.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current prescription refill policy and document clearly on calls.
• Schedule appointments and arrange transportation as needed.
• Check CareReq queue and ensure completion of requests.
• Maintain documentation for all patient requests before ending a call.
• Collaborate to meet departmental metrics and objectives.
• Other duties as assigned.
