Job summary

Optum Home & Community Care, part of UnitedHealth Group, seeks a licensed Nurse Practitioner or Physician Assistant to deliver remote, telehealth care for seniors in nursing homes and assisted living facilities. You will join an interdisciplinary team and coordinate care across settings, with a focus on quality, privacy, and patient satisfaction.

This role offers a Monday–Friday schedule, work-at-home flexibility, and opportunities for professional growth within the Optum care model.

Qualifications

• Active and unrestricted clinical license in the state of residence and Oregon, with ability to obtain in additional required locations.
• MSN or DNP for NPs; active NP license and board certification (e.g., FNP/ANP/ Gerontology/ACNP).
• PA degree; active PA license or ability to obtain by start date; NCCPA board certification.
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
• 3+ years of clinical practice experience.
• Proficient in documenting medical information in EMR systems.

Responsibilities

• Available on telephonic platform to coordinate and manage care for members across caregivers, facilities, hospitals, and providers.
• Use video platform when clinically indicated while maintaining patient privacy.
• Conduct routine, acute, follow-up, and comprehensive assessments.
• Manage members during sub-acute rehabilitation episodes and acute events.
• Ensure accurate and complete documentation of diagnoses (ICD-10) and plan of care.
• Collaborate with interdisciplinary care team and participate in care planning.
