Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to join the Senior Community Care (SCC) team in a remote capacity. The role focuses on telehealth care for patients in nursing homes, senior housing, and assisted living settings, coordinating care across care givers, facilities, and primary care providers.

The position requires board-certified NP or PA, Oregon license, 3+ years of clinical experience, and proficiency with EMRs.

Qualifications

• Active, unrestricted license in your state and Oregon; ability to obtain in others.
• NP path: MSN or DNP + board-certified as NP (AANP/ANCC) in Family/Adult/Gerontology variants.
• PA path: accredited PA degree + NCCPA board certification.
• Current DEA licensure/prescriptive authority where required.
• 3+ years of clinical experience.
• Proficient with EMR documentation.

Responsibilities

• Provide telehealth care coordination for SCC members.
• Assess members via routine, acute, follow-up visits.
• Document care including ICD-10 codes.
• Collaborate with interdisciplinary care team.
• Provide counseling and education to patients and caregivers.
