Qualifications

Responsibilities

• Provide remote telehealth care to patients across settings.

• Conduct routine and advanced assessments and plan care accordingly.
• Manage acute events and sub-acute rehabilitation needs.
• Document thoroughly including ICD-10 coding.
• Engage in care planning and patient/caregiver education.
