Job summary

Optum Home & Community Care is seeking a Senior Community Care clinician to provide virtual, telehealth-based care for patients in nursing homes, senior housing and assisted living settings. This remote role supports an integrated care model and emphasizes patient privacy and collaboration with the care team.

The position requires active NP or PA licensure, 3+ years of clinical experience, and board certification. Expect 40-hour weeks, with daytime Pacific Time hours.

Qualifications

• Active and unrestricted license in the state you reside and Oregon.
• Board certified as NP (AANP or ANCC) with relevant family/gerontology subspecialties.
• 3+ years of clinical experience in practice.
• Current DEA license or ability to obtain post-hire.
• Proficient in documenting medical information in EMRs.

Responsibilities

• Coordinate and manage care for members between care givers, facilities, hospitals, and providers.
• Use video platform when clinical need requires it.
• Perform routine, acute, follow-up and comprehensive assessments.
• Manage members during acute events or sub-acute rehab episodes.
• Ensure accurate ICD-10 documentation and team communication.
• Collaborate with interdisciplinary care team and counsel patients and caregivers.
• Discuss advanced illness and end-of-life planning when appropriate.
