Chapters Health System in the United States is seeking a Resource Nights & Weekend Nurse Practitioner to deliver fully remote serious illness care management to patients and families across our continuum, including home-based, facility-based, and inpatient hospice and palliative care. Qualifications

• Must have a minimum of 3 years of clinical experience in hospice, palliative care, and/or acute inpatient care.
• Must hold a Masters or Doctorate degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and additional states where care is provided.
• Licensure and prescriptive authority must be obtained and maintained for all required jurisdictions. Licensure must be obtained within 60 days of hire.
• Must hold and maintain Basic Life Support certificate and compact RN license in home state by date of hire.

Responsibilities

• Provides fully remote remote virtual serious illness care management to patients and families across Chapters Health System’s continuum of care.
• Consults with medical staff to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medication and equipment as appropriate.
• Supports discharge planning including prescribing medications and entering discharge orders.
• Participates in Quality Assurance/Performance Improvement activities.
