Job summary

ComplexCare Solutions, a national home and virtual assessment provider, seeks a Nurse Practitioner per diem to cover Chesterfield County, VA. You will conduct in-home assessments, document findings, and coordinate with PCPs and health plans to keep members safely in the home and out of the hospital.

The role requires an active NP license, NP certification, CPR, and familiarity with CMS, HEDIS, and EHRs. Telehealth and home visits may be part of the schedule, with about 25 slots monthly.

Qualifications

• Active unencumbered nursing license.
• ANCC or AANP board certification as a Nurse Practitioner or Clinical Nurse Specialist in Family, Adult, Gerontology or Emergency Medicine.
• Maintains current CPR certification.
• Compliance with Healthcare Personnel Vaccinations per policy (Hepatitis B, Flu, MMR, Tdap, Varicella, TB).
• Home health experience a plus.
• Ability to communicate effectively with elderly and chronically ill patients and families.
• Understanding of Medicare/Medicaid and health plan benefit structures.

Responsibilities

• Conduct comprehensive home assessments and virtual evaluations.
• Document and communicate assessment findings, care plans and follow-up needs.
• Coordinate with physicians, PCP and health plans for timely care.
• Ensure CMS, DHH, HIPAA compliance in all records.
• Maintain up-to-date licensure, certifications and health requirements.
• Provide patient education to improve chronic condition management.
