Bayshore HealthCare is seeking a Nurse Navigator for Carepath Digital Health to support clients with chronic disease and elder care remotely. This part-time role includes coordinating second opinions, delivering health coaching, and helping families access appropriate care pathways.

The ideal RN has at least five years of clinical experience, strong communication skills, and proficiency with standard MS Office tools. Remote work with flexible hours is offered.

Qualifications

• Current registration in good standing as a Registered Nurse in the province of practice.
• Minimum five years of proven clinical experience in managing clients with both acute and chronic medical conditions.
• Bilingual in French is considered an asset.

Responsibilities

• Assess the client’s program needs and coordinate second opinion consults.
• Collect information and records for second opinions and post-consultation conference.
• Provide health coaching using motivational interviewing to support goal setting.
• Document all client-related activities in the electronic database and ensure data accuracy.
• Collaborate with clients, families and Carepath team to improve care outcomes.
