Job summary

Nolia Health is seeking a Care Navigator to join our transdisciplinary team, providing virtual, biopsychosocial support to patients with aging-related chronic conditions and to their families.

You will deliver care via telehealth, coordinate with MDs, neuropsychologists, nurse practitioners and social workers, and connect families to local resources while upholding privacy and compassionate, culturally responsive care.

Qualifications

• At least 2 years in healthcare or community-based settings.
• Experience understanding needs of family caregivers and older adults.
• Strong cultural humility and ability to interact with diverse groups.
• Experience with Electronic Health Records (EHR) is a plus.
• Excellent communication and time-management skills.
• Proficiency in another language is a plus.

Responsibilities

• Join a transdisciplinary team to support patients and families.
• Serve as primary contact for caregivers, families and support networks.
• Provide biopsychosocial assessments and develop personalized care plans.
• Coordinate with primary care and specialists to ensure continuity of care.
• Deliver care virtually via telehealth.
• Represent Nolia Health at community events and healthcare partner organizations.
