Job summary

Cadence Health is hiring a Nurse Practitioner to be the primary clinical contact for patients navigating their remote care journey. You will lead decisions on GDMT for heart failure and adherence to guidelines for T2D and hypertension, with asynchronous titration based on vitals and trends.

The role requires clinical autonomy, collaboration with RNs and care teams, and the ability to deliver patient-centered remote care in a fully remote, high-productivity setting.

Qualifications

• Active Compact Multi-State RN license.
• Active NP licensure in at least two states (TX, MI, NC, CA, PA, NJ, CT, AL).
• 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D).
• 3+ years of cardiology experience focusing on heart failure and GDMT.
• Master's Degree as a Nurse Practitioner with willingness to expand licensure as Cadence adds markets.

Responsibilities

• Manage a panel of patients receiving remote care, making timely clinical decisions based on evidence-based guidelines.
• Deliver high-quality, evidence-based care by following established clinical protocols and workflows.
• Initiate and titrate guideline-directed medical therapy (GDMT) with care teams to optimize outcomes.
• Respond to clinical escalations from remote patient monitoring.
• Provide synchronous and asynchronous patient care including reviewing data and conducting virtual visits.
• Meet productivity and documentation expectations.
• Adapt to evolving workflows, software enhancements, and operational processes.
• Collaborate across clinical and operational teams to improve patient care and scalability.
• Help shape the future of a growing digital health organization.
