Cadence is a clinical AI company focused on automating the treatment of chronic disease. This remote NP role centers on managing heart failure, hypertension, and type 2 diabetes through GDMT and secure virtual care.

You will collaborate with physicians, RNs, and care teams, leveraging AI-assisted tools to enhance decision making and patient outcomes while operating in a fully remote setup across multiple states.

Qualifications

• 5+ years experience treating chronic diseases (CHF, Hypertension, T2D).
• 3+ years of cardiology experience with a focus on heart failure management.
• Active ANCC or AANP certification.
• Master's Degree as a Nurse Practitioner.
• Active Compact Multi-State RN license.
• Active NP licensure in at least two of: TX, MI, NC, CA, PA, NJ, CT, AL.
• Stable home office Wi-Fi setup.

Responsibilities

• Manage a panel of patients receiving remote care and make clinical decisions for heart failure, hypertension, and type 2 diabetes.
• Initiate and titrate guideline-directed medical therapy (GDMT).
• Respond to clinical escalations from remote patient monitoring, including abnormal vitals and lab results.
• Provide synchronous and asynchronous care via secure messaging and virtual visits.
• Collaborate with physicians, RNs, and care teams to optimize patient outcomes.
• Use multiple technology platforms and provide feedback to improve clinical operations.
