Job summary

Local Infusion is seeking a skilled Nurse Practitioner licensed in Connecticut to join our virtual care team in a remote, patient-facing role focused on neurology and memory care. You will provide telehealth care, collaborate with physicians, educate families, and document care in the EMR per regulatory standards.

The position emphasizes independent patient management with physician collaboration, ongoing professional development, and a strong commitment to patient access and timely treatment.

Qualifications

• Active, unrestricted NP license in Connecticut.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians.
• Partner with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate clinical documentation in the EMR in line with regulatory requirements.
